The Service
What is the service?
The Learning Disability Team offers a city wide Social work & Care Management Service for people with a Learning Disability and their Carers. The service is delivered in close collaboration with colleagues in Health, as well as the Voluntary and independent Sector, and there is active participation of service users and carers, which is facilitated by the Portsmouth Partnership Board and Portsmouth Carer’s Forum.
The Team is based at The Kestrel Centre and is co-located with our Health colleagues.

What is the role?
This role involves assessing the needs of vulnerable adults with Learning Disabilities and their carer's, to then identify creative ways to meet their needs. The team is committed to pursuing the personalisation agenda, in order to promote independence, inclusion, choice and rights of service users & their carer's. The safeguarding of adults is an important area of your work as will be the implementation of the safeguarding policy and procedures.
Qualified Social Workers are expected to complete continuing health care and mental capacity assessments.
You will be working independently and be responsible for managing your own caseload and maintaining case records within an environment offering a high level of supervision and support.
You will also be undertaking supervisions to support unqualified staff and less experienced staff. You will also be expected to be responsible for your own personal learning and development and cover a duty roster.
Who is the person?
You need to have:
1. Hold a qualification in social work i.e. Dipsw, CQSW, CSS. (Qualified Social Worker Only)
2. Be registered with the GSCC as a social worker. (Qualified Social Worker Only)
3. Be experienced at working with people with Learning Disabilities and have an understanding of statutory services.
4. Experience of working within the sphere of Safeguarding Adults would be an advantage. (Qualified Social Worker Only)
5. Be able to advocate for users and carers using a problem solving approach.
6. Have good assessment skills
7. Knowledge of Valuing People with particular reference to Person Centred Planning, individualised budgets and direct payments
8. Be able to cope with a case workload efficiently and effectively.
9. A good communicator who is able to deal confidently with service users and agencies.
10. Be able to work on your own initiative and be a wider team player who is willing to cover and support colleagues.
